In my previous role as a school nurse, I have developed care plans for students with chronic illnesses and special health needs. I would meet with the student, their parents, and any relevant healthcare providers to gather information about their condition and needs. I would then create a comprehensive care plan that addressed their specific needs and ensured they received the necessary support at school. I would regularly review and update these care plans to ensure they remained relevant and effective. Additionally, I would collaborate with teachers and staff to ensure they were aware of the student's condition and any necessary accommodations. Overall, my experience in developing care plans has allowed me to effectively support students with chronic illnesses and special health needs.
