In my previous role as a Health Information Manager, I encountered a complex issue with an electronic health record (EHR) system. The system was not accurately recording patient diagnoses, which was causing discrepancies in the billing process and impacting reimbursement. To resolve this issue, I formed a team consisting of IT professionals, coding specialists, and medical staff. We conducted a thorough analysis of the problem, examining the EHR system, the coding processes, and the workflow of the medical staff. Through this analysis, we identified several areas that needed improvement, including the training of medical staff on accurate coding practices and the appropriate utilization of the EHR system.
