Summary of the Role
The Geriatric Case Manager will be responsible for coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centered. Handling case assignments, draft service plans, review case progress, and determine case closure help to ensure the elderly are able to stay in their homes and receive the medical and social attention they require.

Qualifications
Bachelor's degree in social work, gerontology, nursing, or a related field.
2-5 years of experience in case management, preferably in geriatrics or a related field.
Current certification as a Case Manager preferred.
Understanding of the aging process, as well as mental health and social services for older adults.
Knowledge of local, state, and federal resources for the elderly.
Ability to work independently and within a multidisciplinary team.
Responsibilities
Conduct comprehensive assessments of client's needs, including social, emotional, physical, and environmental factors.
Develop, implement, and monitor individualized care plans in consultation with healthcare professionals.
Coordinate and facilitate access to services such as medical care, social activities, and other support systems.
Act as a liaison between clients, families, and healthcare providers to ensure communication and proper delivery of care.
Manage and monitor medications and treatment schedules to ensure compliance.
Provide counseling and support to clients and their families regarding issues of aging and long-term care options.
Maintain detailed records of client's care, condition, progress, and problems to report and discuss with supervisors and other team members.
Advocate for client's rights and needs to ensure maximum independence and quality of life.
