Summary of the Role
As a senior community health nurse, you will be responsible for providing comprehensive nursing services to community members, with a focus on preventative care, education, and health promotion. You will have a key role in addressing a wide range of health issues, collaboration with healthcare teams, and advocating for patient care within the community.

Qualifications
Valid nursing license and BSN from an accredited nursing program.
Minimum of 5 years of nursing experience, ideally in a community health setting.
Experience in public health, health education, or related field.
Current CPR certification and additional specialty certifications as required.
Responsibilities
Performing patient assessments and providing direct care to clients in various community settings.
Developing, implementing, and evaluating care plans tailored to individual patient needs.
Conducting community outreach programs, health screenings, and educational seminars to promote health and wellness.
Collaborating with other healthcare providers, community leaders, and organizations to identify and address public health issues.
Acting as a mentor and supervisor to junior nurses and nursing students.
Maintaining accurate and detailed records of patients' health and interventions.
Advocating for vulnerable populations within the community and facilitating access to health resources.
