Experience in care coordination and case management.
Responsibilities
Conduct comprehensive assessments of clients' needs and develop personalized care plans.
Coordinate with healthcare providers, legal services, and community resources to ensure clients receive appropriate support.
Provide counseling and support to clients and their families regarding age-related challenges.
Advocate for the rights of elderly clients and promote awareness of elder care issues in the community.
Assist with the arrangement of services such as home healthcare, transportation, and meal programs.
Monitor clients' progress and adjust care plans as necessary.
Educate clients and their families on available resources and entitlements.
Maintain detailed records of client interactions and services provided.
