Summary of the Role
The Nurse Case Manager is a registered nurse responsible for coordinating all aspects of patient care, including assessment, planning, implementation, and evaluation of individual patient needs. This role requires strong clinical judgement, excellent communication skills, and the ability to advocate for the patient within the healthcare system. The position involves collaboration with other healthcare professionals to ensure high-quality and cost-effective care.

Ability to work independently and as part of a multidisciplinary team.
Advanced understanding of medical terminology and healthcare services.
Responsibilities
Assess new and returning patients to determine their needs and develop appropriate care plans.
Coordinate with healthcare providers, specialists, and other nurses to implement comprehensive care plans.
Facilitate patient access to necessary services, including medical tests, therapies, and educational resources.
Monitor patient progress and outcomes, adjusting care plans as required for optimal results.
Ensure compliance with healthcare regulations and policies.
Provide support and education to patients and their families regarding medical conditions and treatment plans.
Document all patient interactions, treatments, and outcomes in the patient's medical record.
Advocate for patient needs and rights within the healthcare system.
Participate in multidisciplinary team meetings to discuss patient care and potential improvements to care.
Lead initiatives to improve patient care and resource utilization.
