Qualifications

Active Advanced Practice Registered Nurse (APRN) credentials.
Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification.
Demonstrated ability to communicate effectively with patients, families, and healthcare teams.
Knowledge of palliative care principles, techniques, and procedures.
Ability to work independently and as part of an interdisciplinary team.
Responsibilities
Provide comprehensive assessment and diagnosis of patients with life-limiting illnesses.
Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary healthcare team.
Manage symptoms and pain through appropriate interventions and medication management.
Facilitate advance care planning discussions and ensure that patient wishes are respected and documented.
Provide emotional support and counseling to patients and their families.
Educate patients, families, and healthcare providers about palliative care principles and practices.
Collaborate with other healthcare professionals to provide a holistic approach to care.
Monitor patient progress and adjust care plans as necessary.
Participate in continuous quality improvement initiatives to enhance patient care.
Maintain accurate and timely documentation of patient care.
