Qualifications

Responsibilities
Perform comprehensive assessments of patients to determine the need for acute and long-term healthcare services
Develop and implement individualized care plans in collaboration with the healthcare team
Monitor patients' health and progress, adjusting care plans as necessary
Prescribe medications and recommend over-the-counter solutions as appropriate
Coordinate with other healthcare professionals to ensure continuity of care
Lead and participate in team meetings to discuss patient care and outcomes
Educate patients and their families on health maintenance and disease prevention
Provide end-of-life care and support for patients and their families when needed
