Responsibilities
Conduct comprehensive assessments of client needs and risks.
Develop and implement individualized care plans.
Provide ongoing support and case management to clients.
Collaborate with other professionals and agencies to coordinate services and resources for clients.
Maintain accurate and timely records and case notes.
Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
Advocate for clients' rights and access to services.
Conduct home visits and other outreach activities as needed.
Participate in multidisciplinary teams and case conferences.
