Summary of the Role
As a Community Health Nurse Practitioner, you will be responsible for providing primary healthcare services in community settings. You will work with individuals, families, and groups to improve overall health and wellness through preventive care, health education, and direct clinical services.

Qualifications
Active and unrestricted nursing license in the applicable state.
Master's degree in Nursing (MSN) or a Doctor of Nursing Practice (DNP), with a focus on community health.
Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) from a recognized credentialing body.
CPR and Basic Life Support (BLS) certification.
Intermediate experience in a nursing or community health role (2-5 years).
Eligibility to prescribe medication in the state of practice.
Familiarity with community health issues and resources.
Proficiency with electronic medical records (EMR) and telehealth systems.
Responsibilities
Conduct thorough patient assessments, including physical examinations and patient history.
Develop and implement care plans tailored to individual patient needs and community health concerns.
Provide a full range of healthcare services, including diagnosing and treating acute and chronic conditions.
Prescribe and manage medications and other therapies, in compliance with medical guidelines.
Collaborate with other healthcare professionals to ensure a continuum of care.
Educate patients and community groups on health promotion, disease prevention, and lifestyle changes.
Advocate for the health and well-being of individuals and communities.
Monitor and report on the outcomes of interventions and treatments.
Maintain accurate and confidential patient records.
Participate in research and apply evidence-based practice in community health initiatives.
