Summary of the Role
We are seeking a compassionate and dedicated Hospice and Palliative Medicine Physician to join our interdisciplinary team. In this role, you will provide specialized medical care to patients with serious illnesses, focusing on relief from symptoms, pain, and stress in order to improve quality of life for patients and their families.

Qualifications
Medical degree (MD or DO) from an accredited institution.
Valid state medical license and board eligibility or certification in Hospice and Palliative Medicine.
Completion of a residency in internal medicine, family medicine, or a related field, followed by a fellowship in Hospice and Palliative Medicine.
Proven experience in palliative care within a hospice setting or similar healthcare environment.
Demonstrated commitment to providing patient-centered care.
Strong communication and interpersonal skills to interact with patients, families, and healthcare teams.
Responsibilities
Provide comprehensive assessment and treatment of patients with serious illnesses to alleviate suffering.
Develop individualized care plans in collaboration with an interdisciplinary team including nursing, social work, and chaplaincy.
Coordinate with other healthcare providers to ensure continuity of care.
Communicate effectively with patients and families about illness trajectory, treatment options, and care goals.
Lead family meetings to discuss end-of-life care and support decision-making processes.
Prescribe medications and other treatments to manage symptoms such as pain, nausea, and shortness of breath.
Participate in the professional education of team members and contribute to the advancement of the field through research and quality improvement projects.
